It was issued in two formats: 4000 copies with a slip-case (and no dustwrapper) - which went out-of-print almost immediately - and a subsequent case-bound issue with a dustwrapper, which went the same way soon after. The former had a photographic portrait laid into its upper board and the slipcase; the latter was case-bound in plain cloth. This copy is bound as for the slipcased issue, but comes with the dustwrapper.
